ALL CAMP ATTENDEES (campers, counselors, co-counselors, munchkins, activities & staff) must be registered per Camp Oakhurst policy. **NOTE: Registration Forms MUST be submitted on paper/hard copy; emailed versions will NOT be accepted. Campers age limit is now 17.

Here’s what to do:

Print and complete the attached Registration Form. Remember to include:

• Registration Form (3 pages)
• Rules and Regulations
• Emergency Information and Release Form (2 pages)
• A copy of your Medical Insurance Card
• A copy of your Immunization Record
